Diagnosis:

This is a dentigerous cyst for 405 resulting in marked osteolysis and expansion for the right rostral mandible, and loss of bone distal to 404.
Both 406 and 407 may have been missing or may have been lost due to the osteolysis. The crown remnant for 405 is within the cyst.

Treatment:

The best treatment option is excision of the cyst and its lining followed by placement of a canine bone bank origin allograft to help regenerate bone to regain right rostral mandibular strength and to maintain 404.
